When you submit information through FiloCare, you grant us permission to use this information for all purposes permitted under our Privacy Policy, including, without limitation, to match you to providers, coordinate your care across treating providers, and submit claims to relevant insurance companies and other transmitting intermediaries (insurance clearing houses, software services that may interface directly with insurance companies and/or clearing houses).

You are solely accountable for their legitimacy and accuracy, and are solely responsible for legal and financial repercussions of submitting fraudulent, illegitimate, or inaccurate information that you provide. Insurance fraud is a Federal crime.

In using our site, including for purposes of filing reimbursement claims and looking up benefits, you indemnify us against financial loss real or unrealized, liabilities, claims, demands and expenses, including attorney’s fees that may result from your use of our site. The foregoing indemnification obligation does not apply to liabilities, claims and expenses arising as a result of our own intentional misconduct.

9. Geographic Limitations

FiloCare is owned and operated within the United States, and the company intends its Content to be used by persons within the United States. We do not promote the use of the website outside the United States. FiloCare’s services are only for use in the state or states in which the providers listed on the site are licensed. If you see a provider through FiloCare, you represent that you are located in one of the states listed in the Provider’s FiloCare’s profile as a state where the provider is licensed.

10. SMS/Text Notifications

By providing your phone number to FiloCare, you agree to receive communications via phone call and/or text message, which may include calls/texts sent using an automated system, from or on behalf of FiloCare and/or your FiloCare-affiliated provider about account registration, your appointment, rebooking cancelled appointments, insurance claim status, insurance and payment information, and treatment.

10.2 SMS/Text Marketing Messages

As part of the Services, you may receive communications through the Services, including messages sent via email or SMS. By signing up for the Services and providing us with your wireless number, you confirm that you want FiloCare to send you information that we think may be of interest to you. FiloCare may use automated dialing technology to text you at the wireless number you provided, and you agree to receive communications from FiloCare.

In addition to service-related communications, if you opt in to receive marketing messages, you agree to receive automated promotional communications via text message from FiloCare. These may include appointment reminders, provider suggestions, special offers, discounts, newsletters, and other marketing communications. Message frequency varies. Message and data rates may apply.
